Application Guidelines

AWARD DESCRIPTION

The Crest Oral-B/ADHA Clinical Dental Hygienist Award recognizes and honors clinical dental hygienists who make a difference in the lives of their patients/clients.

GUIDELINES

Award recipient will be notified in March 2017 after judges review has been completed. Presentation to the award recipient will be made at ADHA’s 94th Annual Conference in Jacksonville, FL, during the Saturday General Session on June 17, 2017.

DESCRIPTION

This award recognizes and honors clinical dental hygienists who make a difference in the lives of their patients/clients.

AWARD

If your submission is selected you will receive the following award benefits:

• $2,000 cash award
• An engraved award
• Recognition in Access Magazine, ADHA's official publication
• Recognition during ADHA's 94th Annual Conference in Jacksonville, FL which includes:
• Complimentary meeting registration for Annual Conference
• Round trip domestic airfare to Jacksonville, FL
• 2 Days Per Diem ($200 total) to be used in Jacksonville, FL
• $100 for ground transportation in Jacksonville, FL
• 2 Nights Hotel Accommodations in Jacksonville, FL
• Award recognition during Saturday's General Session, June 17, 2017

JUDGING

The Crest Oral-B/ADHA Clinical Dental Hygienist Award Committee will conduct award judging.

REQUIREMENTS

• Applicant must be an U.S. citizen who is licensed dental hygienist operating in a clinical dental setting.
  
• Applicant must be a current member of the American Dental Hygienists’ Association.
  
• Current members of ADHA’s Board of Trustees, councils, employees, and family members of these individuals are not eligible for this award.
  
• Employees of Procter & Gamble and their family members are not eligible to apply for this award.

SUBMISSION OF ENTRY

• All materials sent to the Crest Oral-B/ADHA Clinical Dental Hygienist award competition must be typed in the format set up in the document. (Applications not typed will be disqualified.)

• All entries must be accompanied by a signed application. The deadline for the 2017 Crest Oral-B/ADHA Clinical Dental Hygienist Award is Wednesday, February 1, 2017.

• Only the material requested will be allowed. Additional items such as videocassette tapes, brochures, and photographs will be discarded and not returned. All entries are non-returnable, and remain the property of ADHA.

• An individual may submit only one application during the course of the award year, and, if not funded, may not resubmit until a subsequent competition. More than one entry from an applicant will not be allowed.

• The decision of the award review committee will be final. ADHA and Crest reserve the right not to distribute grant monies if funding is not available or a wide pool of applicants cannot be found.

• Semi-finalists in the competition may be asked to submit supporting documentation. ADHA will maintain strict confidentiality in regard to this information.
